Preparing myself for Ultra Hard I tried to get most of the Adept weapons and outfits but couldn't get hearts from Stormbird, Thunderjaw, and Behemoth.
The icon that looks like a shield with a tear or rip in it is Tear damage. That impacts how much damage the weapon/ammo does to armor, in terms of tearing off pieces of armor. You'll probably care about this only on Tearblast arrows, which are made for armor removal. The third icon, if present, is an elemental damage type - Blast, Fire, Shock

As far as tips go, personally I was surprised how easy I found NG+ UH. However I’m sure this is down to the prep I did. Most important thing would be to look up the adept weapon requirements and farm BEFORE you start NG+, they take a lot of shards ~33k and hearts so get what you want first. For recommendations I’d say a must is the adept
